COORDINATION SERVICES MANAGER - BILINGUAL SPANISH/ENGLISH

 

JOB SUMMARY:

Supervise Coordinators in their daily operations.  Ensure that professional relations exist with Coordinators, Patients, and Home Health Aides.  Ensure that Coordinators are properly instructing the Home Health Aides about the specific needs of the clients and that services are provided as prescribed by the client’s Plan of Care.  Provide continuous home care services for the clients by assigning Home Health Aides who have fulfilled all necessary contractual and regulatory requirements, i.e. in-service training (State, Federal, and contractual regulations). 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1.      Must be competent with the essential duties and responsibilities of the Coordinator and Case Acceptance Coordinator.

2.      Supervises the activities of the coordinating team in providing continuous home care services for the clients.

3.      Monitor adherence, of all staff to contractual and government regulatory guidelines.

4.      Provide corrective action for staff and Home Health Aides as needed.

5.      Ensure that professional relations exist with Coordinators, Patients, and Home Health Aides.

6.      Promote teamwork environment.

7.      Maintain efficient methods to ensure daily responsibilities are carried out routinely by staff for patient’s continuity of care.

8.      Report major incidents to Associate Administrator and or Administrator.

9.      Evaluate Coordinators performances.

10.   Complete other task assigned by Associate Administrator and Administrator.

11.     Implement awareness of the company’s mission, vision, values and goals.

12.     Update staff to all Company changes and announcements.

 

Requirements

QUALIFICATIONS/EDUCATION:

1.      High school diploma or equivalency required.  Two years of College preferred.

2.      Three years of general office experience required, preferably in a health care or customer service setting.  Supervisory experience preferred.

3.      Excellent leadership and problem solving skills.

4.      Excellent communication, analytical, negotiation and interpersonal skills required.

5.      Demonstration of good listening, oral and written skills required.

6.      Able to operate telephone, PC, copier and other basic business machines is required.

7.      Bilingual Spanish required.
